Service Provider Log List

This program will provide a list of Service Provider Logs.

Menu Location

Special Education > Reports > Service Provider Log List

Setup Options

Year - Defaults to the current year. A prior year may be accessed by clicking in the field and making the appropriate selection from the drop down list.

District - Default value is based on the user's security settings. It will be limited to the user's district only.

School - Default value is based on the user's security settings. If the user is assigned to a school, the school default value will be the user's school site code.

Provider - Click in the box to select provider.

Supervisor - Click in the box to select supervisor.

Service - Click in the box to select service.

Log Start Date - Enter date log began.

Log End Date - Enter date log ended.

Custom Codes - Select Custom Codes created in the Custom Codes Management to filter the student list.

Special Codes - Leave blank if all special codes are to be included into the report. Otherwise, choose the desired special code.

Program - Leave blank if all program codes are to be included into the report. Otherwise, choose the desired program code.

Print Break On - Click inside the box to choose option. Default is "None".

Report - The user can click in this box to select a report that was previously created.

OK - Click to continue

Column Headers

Sch - Student's school of enrollment

Student Name - Student's full name

SIDNO - Student's identification number

Service - Service code and description

Provider ID - Service provider's identification number

Provider Name - Service provider's name

Supervisor ID - Service supervisor's identification number

Supervisor Name - Service supervisor's name

Log Start Date - Date and time service log started

Log End Date - Date and time service log ended

DX Code - Diagnosis code

CPT Code - Medicaid CPT code

Service Status - Status of the service provided

Setting Type - Setting type

Location - Location (64 characters)

Supervisor Approve - If a supervisor creates a service log in the Service Provider Log, this box in the column will automatically be checked.

% Achieved - Percentage achieved.

Short Term Obj # - Short term objective number.

Notes - Log notes

Setup - Click to go back to the Setup box.

Print - This will allow the user to print the report.

Help - Click to view written instructions and/or videos.

Save - Click save first and a box will appear to name the report. Then click to save the report.

Report - The user can click in this box to select a report that was previously created.
