Personal Care Service Log

This program will allow users to enter Personal Care Services given to students.

Menu

On the left navigation panel, select Health > Entry > Personal Care Service Log.

Setup Options

Year - Defaults to the current year. A prior year may be accessed by clicking in the field and making the appropriate selection from the drop down list.

District - Default value is based on the user's security settings. It will be limited to the user's district only.

School - Default value is based on the user’s security settings. If the user is assigned to a school, the school default value will be their school site code.

Grades - Click inside the box to select student's grade of enrollment.

Start Date and End Date - A set of dates that limits the selection of records for the report to a beginning date (from) and an ending date (to) range.

Print Break on Student - Check this box to print each student seperately.

Ok - Click Ok to continue.

Column Headers

D - Delete this record only.

Student Name - Student's full name.

SIDNO - Student's school identification number.

School - Student's school of enrollment.

Grade - Student's grade of enrollment.

Service - Service provided.

Log Start Date - Log start date and time.

Log End Date - Log end date and time.

Provider - Service provider.

Note - Notes

Create Who - User who created the record.

Create Date - Date the record was created.

Change Who - User who changed the record.

Change Date - Date the record was changed.

Bottom

Setup - Click to go back to the Setup box.

Print - This will allow the user to print the report.

Help - Click to view written instructions and/or videos.
