Demographic Form

Print - Check either box With SSN or Without SSN.

Check any of these options to show on the Demographic Form.

• Standard - Standard will show the standard information. Note that regular Special Codes and Lunch Special Codes will show with this setting.
• Bus - Bus will show slightly different information with bus information at the bottom.
• Show Special Codes
• Show Additional Comments
• Show Custom Codes
• Show Can Pick Up

Custom Codes - This option limits the visible custom codes on the printout to only those that are selected. This field will be disabled if the Show Custom Codes box is not checked.
