The History - Procedure/Treatment tab is to allow nurses to enter information containing procedure/treatment for students. Data is entered in the fields from left to right.
Column Headers
To find definitions of commonly used column headers, follow this link: Common Column Headers.
For other columns not listed in the common column list:
- Start Date - Start Date for the Procedure or Treatment
- End Date - End Date of the Procedure or Treatment
- Procedure/Equipment - The Equipment needed to Take Care of the Procedure
- Time(s) for Procedure - The Time(s) of the Procedure
- Instructions - Instruction Regarding the Procedure of Treatment
- Provider - The Health Care Provider and Contact Number
- ICD Code - This is the Diagnosis Code
- Reason - This is the Reason for the Procedure
- Comment - Nurses Notes and/or Comments about the Procedure
- Nurse - This is the Nurse that entered the information
Add New Doctor - Add the Doctor and Contact Phone Number for the Procedure
