The History - Procedure/Treatment tab is to allow nurses to enter information containing procedure/treatment for students. Data is entered in the fields from left to right.


Column Headers

  • Start Date - Start Date for the Procedure or Treatment
  • End Date - End Date of the Procedure or Treatment
  • Procedure/Equipment - The Equipment needed to Take Care of the Procedure
  • Time(s) for Procedure - The Time(s) of the Procedure
  • Instructions - Instruction Regarding the Procedure of Treatment
  • Provider - The Health Care Provider and Contact Number
  • ICD Code - This is the Diagnosis Code
  • Reason - This is the Reason for the Procedure
  • Comment - Nurses Notes and/or Comments about the Procedure
  • Nurse - This is the Nurse that entered the information

Add New Doctor - Add the Doctor and Contact Phone Number for the Procedure
