  • Parent/Guardian Address Information
  • Click Continue

Screen Two

  • Username
  • Password
  • Confirm Password
  • Click Continue

Screen Three

  • Security questions and answers
  • Click Continue

  • Paren/Guardian Information
    • Relationship
    • First Name
    • Last Name
    • PSN Number
    • Cell Phone
    • Email
  • Click Continue
