SIDNO - Student's school identification number.
Service - Click in the box for a list to choose from.
Provider - Service provider's name.
Log Start Date - Log start date and time.
Log End Date - Log end date and time.
Note - This is not a required field, but any documentation can be added.
Save - Click to save new or changed information.
Cancel - Click to close this box.
