Immunization Report

This report will allow a nurse to run a report for students that have Immunizations listed in the Health System.

Getting Started

  1. In the left navigation panel, click Health, then click on Lists, then Immunization Report.

Setup Options

  1. Year: Defaults to current year.
  2. District: Defaults to district number associated with the login of the user.
  3. School: Click to select the desired school.
  4. Show SSN on Reports: Leave the check box blank is the SSN is desired for the report. Place a check in the check box, if the SSN is desired for the report.
  5. Grade: Click to select the grade level(s) of students to be listed.
  6. Race: If desired, the user can select to limit the listing of students by Ethnic code. If left blank, all ethnic codes will be included.
  7. Gender: Click to limit the listing to students to a selected gender. If left blank, all students, regardless of gender, will be listed on the screen.
  8. Inactive: Active is the default. If this selection needs to be changed to Inactive or All, click in the cell and make the desired choice.
  9. Exempt: This cell defaults to All. This may be changed by clicking in the cell to bring up a pick box. Make your desired select from this box by click on your choice.
  10. Complete: This cell defaults to All. This may be changed by clicking in the cell to bring up a pick box. Make your desired select from this box by click on your choice.
  11. Vaccine: The date defaults to today`s date. This may be changed by clicking on the down arrow(s) or by clicking on the calendar to far right
  12. Vaccine: The date defaults to today`s date. This may be changed by clicking on the down arrow(s) or by clicking on the calendar to far right
  13. Past Due: The date defaults to today`s date. This may be changed by clicking on the down arrow(s) or by clicking on the calendar to far right
  14. Click the OK button to begin the Asthma Report.
