This report will give student data for the Plan Date List.

Setup Box Options

Additional Choices in the Main Setup Box

1. Exceptionality-Click in the cell to the right of Exceptionality and an Exceptionality box will populate-make your choice by clicking the square to the right. More than one may be chosen.
2. Placement-Click in the cell to the right of Placement and a Select Placement box will pop up. Make the choice of one or more by clicking in the square to the left of the corresponding number that represents the description of the placement.

Make the choices from the following list in the setup box by clicking in the square located to the left of the users` choice(s).

1. Exclude students with no exceptionality
2. Show Health Care Plans Only
3. Exclude Gifted/Talented students
4. Show Behavior Plans Only
5. Double Space Print

Click OK to run the report.

Column Headers

1. Student Name-Name of student
2. Sch-School number that the student is enrolled
3. Eth-Ethnicity of student
4. Gen-Gender of student
5. GD-Grade of the student
6. Age-Age of student
7. Birth Date-DOB of student
8. Except-Primary Exceptionality
9. SEC EXC-Secondary Exceptionality
10. Eval-Last evaluation date
11. IEP DT-Last IEP update
12. IEP Teacher-IEP holder for the student
13. Ser-Supplemental services
14. Health-Health Plan date
15. Behavior-Behavior Plan date

Setup Button-Enter the information need for the report(s)

Click OK to run the report.
