Incident and Action Taken Matrix

- Log into WebPams page
- On the left, click on Discipline
- Under Discipline, click on the arrow to the left of the word List
- Click on Incident/Action Taken Matrix that appears under List
- Once the setup box appears, the following data should be set appropriately to receive accurate data:
- Year should be set to the school year in which the report data need to be collected from
- District and School should be defaulted based on the users security information
- From Date and To Date both default to today`s date. This may be changed by clicking on the down arrows or by clicking on the calendar at the right of the line.
- Active Only: Check the box for active only students
- 'Show:
- Numbers: Select the radio button the corresponds with the desired data needed
- Type: Select the radio button the corresponds with the desired data needed either Numbers or Percentage
- Incident Codes: The cell may be left blank to select all codes or click in the cell to make a selection of a particular code(s)
- Show Remote Scheduled Only: Check the box if only remotely scheduled are to be shown.
- Click the OK button to begin the Incident and Action Taken Matrix
Definitions of Column Headers
- Incident: Incident Code and Description
- 01 White: Referrals for Admin Action `01 Held Conference with Student for Ethnicity `White`
- 01 Black: Referrals for Admin Action `01 Held Conference with Student for Ethnicity `Black`
- 01 Hispanic: Referrals for Admin Action `01 Held Conference with Student for Ethnicity `Hispanic`
- 01 Asian: Referrals for Admin Action `01 Held Conference with Student for Ethnicity `Asian`
- 01 Native American: Referrals for Admin Action `01 Held Conference with Student for Ethnicity `Native American`
- 01 Pacific Islander: Referrals for Admin Action `01 Held Conference with Student for Ethnicity `Pacific Islander`
- 02 White: Referrals for Admin Action `02 Held Conference with Student for Ethnicity `White`
- 02 Black: Referrals for Admin Action `02 Held Conference with Student for Ethnicity `Black`
- 02 Hispanic: Referrals for Admin Action `02 Held Conference with Student for Ethnicity `Hispanic`

Buttons at the Bottom of the Allergies Report
- Setup: Click to go back to the Setup box.
- Print: This button allows the user to print the screen information in a variety of formats. Default is pdf format, but can be changed to xls (Excel), or csv (text).
- Help: This button is NOT activated.
