OSS - Out of School Suspension.

Bus - Bus Suspension.

Code - Code of discipline.

Action Taken by School Administration - Description of discipline code.

Date - Date of action taken.

Points Lost - Amount of points lost due to the referral

Points Earned - Amount of points earned due to the referral

Administrative Comments - Comments made by administration.

Date Disciplinarian Signed Form - Date the disciplinarian signed the form.

Disciplinarian Signed Form - Name of disciplinarian.

Other Agencies - Any other agencies involved in this event.

Alt Site/Program - If student is sent to an alternative site, click to select the Alt Site Program the student will be attending. Required for action codes 11, 32, 33, 70.

Minutes Removed - Number of minutes the student was removed from class and their regular setting.

Third Party Authorized Punishment - Corporal Punishment directly authorized by uninvolved third party tells whether the Corporal Punishment decision was reviewed by a third party and deemed warranted.

Perpetrator Received

Serious Injury - Tells whether the perpetrator received any serious injuries.

Medical Treatment - Tells whether the perpetrator received any medical treatment.

Event Related Too

Hate/Bias - Describes what group was being targeted if this event was motivated by hate or prejudice against a specific group of individuals.

Drugs - Check this box if event was related to drugs.

Alcohol Use - Check this box if event was related to alcohol use.

Gang - Check this box if event was gang related or motivated.

Interventions

  • The user has to have a RTI 108 Behavior Intervention to be able to see the new table.
  • If no 108 interventions are defined then you will not see the table, but only a message: "No behavior (108) interventions are defined for your school/district. Approved intervention choices are defined in the SBLC/RTI Editor, Action>RTI Tier-CAT Editor.

Tier - Referral tier level.

Category - Referral category code.

Intervention - Referral intervention code.

Start Date - Referral start date.

End Date - Referral end date.

Instructor - Instructor assigned to intervention.

Days - Days of the week for intervention.


NOTE:

  • If the referral is a Minor and Code 6 is not checked on Referral Page 2, it will send a call to the parents immediately when the referral is created.
  • If the referral is a Minor and Code 6 is checked on Referral Page 2, no call is made to parents until the Administrative Action has been added to the Admin page.
  • If the referral is a Major, it immediately sends a call to parents when the referral is created.
