- Sign Date: Disciplinarian sign date
- ISS Begin Date: Date ISS starts

- SIS Action: SIS action date
- Admin 1: Admin action Code 1
- Admin 2: Admin action Code 2
- Admin 3: Admin action Code 3
- Admin 4: Admin action Code 4
- OSS Begin Date: Date OSS starts
- OSS End Date: Date OSS ends
- Expel: Displays if a student is expelled
- Transfer: Is student expelled to another school
