This report will allow a nurse to run a report of students with data on the Consultation tab of the Health System.

Getting Started

  1. In the left navigation panel, click Health, then Lists, then Consultations By Date Report.

Setup Options

  1. Year: Defaults to current year.
  2. District: Defaults to district number associated with the login of the user.
  3. School: Click to select the desired school
  4. Date From: The date will default to today`s date. To change the date click on the drop down arrow(s) or click on the calendar to the far right of the line.
  5. Date To: The date will default to today`s date. To change the date click on the drop down arrow(s) or click on the calendar to the far right of the line.
  6. Time From: The time will default to 00:00. The time may be changed by clicking in the cell. Remember to enter "Military" time.
  7. Time To: The time will default to 23:59. The time may be changed by clicking in the cell. Remember to enter "Military" time.
      • Show SSN on Report-Check the box to show SSN on the report.
      • Show Only Medication Consultations-Check the box to show Only Medication Consultations.
      • Print Break on Student-Check the box to Print Break on Student.
  1. Object Function-Allows the user to select an Object Function code to get a count of consultations by this Object Function Code.
  2. Click the OK button to begin the Consultation By Date Report.

Definitions of Column Headers

  1. Student Name: Name of student
  2. Sidno: Student ID number
  3. SSN: SSN for the student if the box on in the Setup has been checked. It will not show if the box is not checked.
  4. Sch: School number
  5. Gender: Students gender
  6. Grade: Grade level of student
  7. DOB-Date of Birth of the student
  8. CPT Code: Medicaid CPT code
  9. CPT Code Description: Will be the medical description for the CPT Code.
  10. DX Code: Medical code for the condition
  11. Reason: Reason for consultation
  12. Action Taken: Action taken by the nurse
  13. Nurse: Nurse that entered the consultation
  14. Late: Late Entry-The consultation was entered for a previous day
  15. Date: Date of the consultation
  16. Start Time: Time of the consultation (military time)
  17. End Time: The number of units that the nurse spent with a student (A unit is defined as being 15 minutes and is used for Medicaid billing.)
  18. Unit-Shows the number of units to be billed for in Medicaid.
  19. Last Name: Student's Last Name
  20. First Name: Student's First Name
  21. Middle Name: Student's Middle Name
  22. SASID: Student's State Assigned Student Identification Number

  1. Setup: Click to go back to the Setup box.
  2. Print: To find the print instructions, follow this link: Standard Print Options.
