This report will allow a nurse to print Emergency Information Forms. The form contains a summary of demographic information, contacts, and medical information on a single page for each student.

Getting Started

  1. In the left navigation panel, click Health, then Lists, then Emergency Information Report.

Setup Options

  1. Year: Defaults to current year.
  2. District: Defaults to district number associated with the login of the user.
  3. School: Click to select the desired school.
  4. Grade(s): Click to select the grade level(s) of students to be listed.
  5. As of Date: This date will default to today`s date. If the user need to change the date, do so by clicking on the drop down arrows or click on the calendar to the far right of the line.
  6. Show SSN on report: The SSN will not show on the if the check box is not checked. To show the social security number on the report check the check box.
  7. Click the OK button to begin the Emergency Information Report.

Definitions of Column Headers

  1. Select Box: Click to select a student to have an Emergency Information Form printed.
    • Note: At the top of the column is a "Select All" box next the "Student's Name" label. Click to quickly select all students in the listed to have an Emergency Information Form printed.
  2. Student Name: Name of student.
  3. Sidno: Student ID number.
  4. SSN: Social Security number of the student.
    • Note: This will show if the user selected Show SSN on the setup box. If this was not selected, the user will see "XXX-XXX-XXX" in the column.
  5. Sch: School number.
  6. Grd: Grade of the student.
  7. Eth: Ethnicity of student.
  8. Gen: Gender of student.
  9. Birth Date: Date of birth.
  10. Preferred Hospital-The Hospital that the guardian prefers in case of emergency.
  11. Doctor`s Name: Name of student`s doctor
  12. Doctor`s Phone: Phone number of student`s doctor
  13. Medicaid/Insurance: Medicaid/Insurance information
  14. stuType: Identifies if the student is a Dorm (lives on campus) or Day student (lives with guardian).
  15. Last Tetanus: Date of the last Tetanus shot
  16. Height: Height of student
  17. Weight: Weight of student

  1. Setup: Click to go back to the Setup box.
  2. Print: Clicking this button will only print the listing of students as shown on the screen. To make the Emergency Information Forms, use the Print Forms button (described below). To find the print instructions, follow this link: Standard Print Options.
  3. Print Forms: Click to print the Emergency Information Form for the selected students.
  4. Help: Click to get instructions.
