Information about a school will appear or be created on the Demographic screen.
At the top of the page is:
- Year - Applicable school year.
- District - School district in which the school belongs.
- School Name - Official school name registered with the Louisiana Department of Education.
- School Number - The last three digits of the site number assigned to the school by the Louisiana Department of Education.
- Established Date - The date the school was officially opened.
- Closed Date - The date the school was officially closed.
- Principal's Title - Dr., Mr., Mrs., etc.
- Principal's First Name - First name of the principal or head of the school.
- Principal's Last Name - Last name of the principal or head of the school.
- Principal's Email Address - E-mail address of the principal or head of the school.
In the next section, select the grade levels that are offered at the school site. The valid grade levels are defined below per the Louisiana Department of Education SIS User Guide:
- 01 – First
- 02 – Second
- 03 – Third
- 04 – Fourth
- 05 – Fifth
- 06 – Sixth
- 07 – Seventh
- 08 – Eighth
- T9 – Transitional Ninth
- 09 – Ninth
- 10 – Tenth
- 11 – Eleventh
- 12 – Twelfth
- 15 – Infant Program: *Students must be enrolled in the Special Education SER system to use this grade
- 20 – Preschool Services: Preschool students, not in a state-approved kindergarten, who are receiving special education services and/or direct/related *services, e.g., speech, OT, PT, etc. * Students must be enrolled and in the Special Education SER system to use this grade
- 24 – Pre-Kindergarten: Regular Education students who are younger than the age required to enter kindergarten in a particular district. *All pre-*kindergarten students must have a Pre-Kindergarten Funding Source Code. These students should not have active IEPs in SER.
