This report will allow a nurse to create a listing of students enrolled by grade or homeroom.
Menu Location
Health > Reports > Student List Report
Setup Options
Year - Defaults to current year.
District - Default value is based on the user's security settings. It will be limited to their district only.
School - Default value is based on the user's security settings. If the user is assigned to a school, the school default value will be their school site code.
Show SSN on Reports - Leave the check box blank is the SSN is desired for the report. Place a check in the check box, if the SSN is desired for the report.
Grade - Leave blank or select all to include all grade levels. Otherwise, choose the desired grade.
Ethnic - Leave blank or select all to include all ethnicities. Otherwise, choose the desired enthnicity.
Gender - Leave blank or select all to include both genders. Otherwise, choose the desired gender.
Inactive - Active is the default. If this selection needs to be changed to Inactive or All, click in the cell and make the desired choice.
Medicaid - All is the default. If this selection needs to be changed from All, click in the cell and make the desired choice.
Birth Month - All is the default. If this selection needs to be changed from All, click in the cell and make the desired choice of month(s.

Column Headers
Student Name - Name of student.
Sidno - Student ID number.
Address 1 and 2 - Student's mailing address line 1 and 2.
City - Student's mailing address city
State - Student's mailing address state
Zip - Student's mailing address zip
Phone - Student's main home phone number
School - School number.
Grade - Grade of the student.
Gender - Gender of student.
Ethnic - Ethnicity of student.
is Sped - Is student sped
is 504 - Is student 504
is LEP - Is student LEP
DOB - Date of birth.
SSN - Social Security Number of student
Immunization ID - The ID for the student in the local Immunization tracking network.
Parent/Guardian - Student's parent/guardian name
Teacher/Counselor - Student's teacher/counselor name
Sasid - Student's state identification number
Guardian's Work - Guardian's work phone number
Mother's Work - Mother's work phone number
Father's Work - Father's work phone number
Mother's Cell - Student's mother's cell phone number
Father's Cell - Student's father's cell phone number
Advisor - Student's current advisor
Team - This selection should only be used if your school is using Team Scheduling. Leave blank if all team names are to be included into the report. Otherwise click in the field to the right of Teams to select the desired Teams(s) to view.
Last Name - Student's last name
First Name - Student's first name
Middle Name - Student's middle name
Morning Bus - Student's morning bus number
Evening Bus - Student's evening bus number
Morning Stop - Student's morning bus stop
Evening Stop - Student's evening bus stop
District of Residentce - Student's district of residence
Transferred From - School student transferred from
Transferred To - School student transferred to
Entry Date - Student's entry date into the school
Leave Date - Student's leave date from the school
Health Alerts - Student has a health issue?
VisScr Date - Student's last vision screening date
HeaScr Date - Student's last hearing screening date
Lunch Code - Student's lunch code
Lunch ID - Student's lunch identification number
T & R - Student's township and range information
Emergency Plan - Does the student have an emergency plan?
Health Comments - Health comments.
