Posting a Corporal Punishment video


How-to for Teachers

  1. Log into Gradebook
  2. On the left of the screen under the heading Entry, click on Gradebook.
  3. At the bottom of the WebGradebook, verify the school year and teacher. Select the Course then marking the period.
  4. Select the student by clicking on the student’s name.
  5. Click the Discipline button at the top of the window. The RefPG1 will be displayed with some information about the student and the incident already filled in.
  6. Verify Code (time code) and Location. Make a new selection from the list if a change is needed from the default code of 02 (during class) and 13 (classroom).
  7. Click the Incident Code cell and select an appropriate code, then click Ok. You may select up to 3 additional incidents by clicking the code field on the remaining blank lines. Select the most important incident code first.
  8. If the incident involves bullying, use this link for instructions on entering the required incident information Posting Bullying. Use the back arrow to return to these instructions.
  9. Click on the RefPG2 tab at the bottom left of the screen to change to page 2 of the referral.
  10. You may enter up to four Actions that you took as the teacher.
  11. If Corporal Punishment is administered select Code 98 . By selecting this action you must specify the following pieces of information:
    • Date
    • Staff Name--Default value is the staff entered on RefPg1 and can be changed by clicking the field, selecting the appropriate staff group at the bottom of the popup list
    • Title--position of the staff administering the corporal punishment
    • SIS Incident--select from the popup list.
    NOTE: The code selected on RG1 may be different from the code selected here. Be sure to select the one with matching description.
  12. If Code 06 Referred to Office is selected the principal and disciplinarians will receive an email notifying them of the new referral.
  13. Select the type of parent contact; enter the date of the contact.
  14. If there are any Recommendations to enter, type them in the Recommendations field, then select the date.
  15. Enter the Motivation for the incident in the Motivation field.
  16. You may enter additional perpetrators of the incident in the Perpetrator field. By selecting additional perpetrator(s) RefPg1 and RefPg2 will be created for these additional students when this first record is saved.
  17. The Admin and Hearings pages will not have any info on them at this time. You do not have permission to enter any data on these pages but you may view the administrative decision later via the gear box for the student.
  18. Click the Save button at the bottom of the screen. Then click the red X at the top right to return to the WebGradeBook.


How-to for Administrators, Counselors, and other Staff

  1. Click on Discipline > Discipline Posting > Entry.
  2. If you need instructions for setup values to be used, follow the link for Standard Setup Options.
  3. OK - Click to continue.
  4. On RefPG1, left mouse click in the Name field and select the student from the alpha list that appears
  5. If this is a bus referral select the check box on the right side of the screen under the student information
  6. Left mouse click in the staff field and select the staff that reported the discipline incident; use the tabs at the bottom of the staff list to change from TEA to ADM, BUS, SCH, DST
  7. Select the date and time of the incident. Be sure to select the CODE and the LOC that best describes the time and location
  8. Select the Incident(s). Select as many as 4 incidents that best describes the behavior of the student
  9. Enter any witnesses to the event using the top line of the Witness section to select each witness
  10. If evidence exists, select it from the dropdown list in the evidence field
  11. Type the remarks describing the incident. NOTE: Only the top 4 lines of the remarks area will print on the state discipline form
  12. Click on the "RefPG2" tab at the bottom of the screen
  13. Select Teacher action(s). NOTE: if action 06 is selected an email will be sent to the school disciplinarian
  14. Select parent contact method and contact date
  15. Type teacher remarks and recommendations
  16. Enter date employee signed the form
  17. Enter motivation for the behavior from the dropdown list
  18. Select any victims if applicable (required for bullying incidents)
  19. Using the top line of the perpetrators section enter additional perpetrators if applicable (after saving the record the name of the student on Pg 1 will also be displayed in the perpetrators list on Pg 2). NOTE: By entering additional perpetrators, Pg1 and Pg2 info will be created for all perpetrators when the first student record is saved
  20. Click on the Admin tab at the bottom of the screen
  21. For corporal punishment select admin action code 12
  22. A new window for corporal punishment information will appear on the screen.
  23. Click in the staff field and make the selection
  24. Click in the title field and make the selection
  25. Click in the incident code field and make the selection
  26. Select up to 3 other actions taken by administrator
  27. Enter administrative comments
  28. Enter the date disciplinarian signed the discipline form
  29. Select administrator entering/signing the form
  30. Enter additional information related to other agencies involved, level of injury if applicable, or event related to hate/bias, drugs, alcohol, or gangs
  31. Enter the Minutes Removed. This is not a required field. Minutes removed is commonly used with partial day administrative action codes. The user will select one the partial day codes and enter the number of minutes they will be removed from instruction. Minutes removed is not calculated based on the time range posted for a student discipline record but only on the minutes entered here.
  32. Click on the Comments tab at the bottom of the referral page
  33. Enter comments made by the student and/or the parent
  34. Enter the date the comments were made
  35. Enter the student or parent’s name that made the comment
  36. Save the referral using the Save button at the bottom of the screen
