This report will allow a nurse to run a report for students that have a Vision Screening listed in the Health System.

Getting Started

  1. In the left navigation panel, click Health, then click on Lists, then Vision Report.

Setup Options

  1. Year: Defaults to current year.
  2. District: Defaults to district number associated with the login of the user.
  3. School: Click to select the desired school.
  4. Show SSN on Reports: Leave the check box blank is the SSN is desired for the report. Place a check in the check box, if the SSN is desired for the report.
  5. Grade: Click to select the grade level(s) of students to be listed.
  6. Ethnic: If desired, the user can select to limit the listing of students by Ethnic code. If left blank, all ethnic codes will be included.
  7. Gender: Click to limit the listing to students to a selected gender. If left blank, all students, regardless of gender, will be listed on the screen.
  8. Active Only: Active is the default. If this selection needs to be changed to Inactive, click in the cell and make the desired choice.
  9. Function-Click in the cell to bring up a list of Object Function Codes and their descriptions. Select the one needed for the vision report for Medicaid payment.
  10. Home Room-Click in the cell to select the homeroom(s) needed for the report.
  11. Pass/Fail/Not Tested: Defaults to All. This may be changed by clicking in the cell and select your choice from the pick box that populated. There may be more than one choice selected from this box.
  12. Referred: Defaults to All. This may be changed by clicking in the cell and select your choice from the pick box that populated. There may be more than one choice selected from this box. Click OK at the bottom of the box.
  13. Medicaid: Defaults to All. This may be changed by clicking in the cell and select your choice from the pick box that populated. There may be more than one choice selected from this box. Click OK at the bottom of the box.
  14. From Date: The date defaults to today`s date. This date may be changed by clicking on the drop down arrow(s) or by clicking on the calendar to the far right of the line.
  15. To: The date defaults to today`s date. This date may be changed by clicking on the drop down arrow(s) or by clicking on the calendar to the far right of the line.

Click OK at the bottom of the box.

Definitions of Column Headers

  1. Student Name: Name of student.
  2. Sidno: Student ID number.
  3. School: School number.
  4. Grade: Grade of the student.
  5. Gender: Gender of student.
  6. Ethnic: Ethnicity of student.
  7. DOB: Date of birth.
  8. SSN: Social Security Number of student
  9. Homeroom: Homeroom teacher of student
  10. Screened: Date the student was screened
  11. Near R: Near right test
  12. Near L: Near left test
  13. AC-R: Vision acuity right
  14. AC-L: Vision acuity left
  15. MF: Muscle function
  16. MB...: Muscle balance
  17. CV: Color vision
  18. Rx: Prescription
  19. GL: Tells if a student needs glasses or not
  20. Tool: Tool that was used to determine sight
  21. +L: Plus lens
  22. Comments: Comments about the screening
  23. Nurse: Nurse that entered the information
  24. RF: Referral
  25. F-UP 1: First follow up
  26. F-UP 2: Second follow up

  1. Setup: Click to go back to the Setup box.
  2. Print: To find the print instructions, follow this link: Standard Print Options.
  3. Help: Click to get instructions.
