Print Student Request Forms

This program will allow the user to quickly print all student`s request forms.

Main

Request forms menu.png On the left, click on Scheduling. Under Lists, select Print Student Request Forms.


Setup Options

For other set up values not listed in the standard set up list:

Year--Defaults to the current year. A prior year may be accessed by clicking in the field and making the appropriate selection from the drop down list.
District--Default value is based on your security settings. You will be limited to your district only.
School--Default value is based on your security settings. If you are assigned to a school, the school default value will be your school site code. You will not be able to change this value. If you are a supervisor or other district office employee with access to the Student Information System, you will be able to run programs for one school, a small group of schools, or all schools in your district.
Student Grades--Grade--Grade level of students whose records need to be edited or whose names are to be included on a report. Leave the field blank if all grade levels are to be included. Otherwise, click in the field to the right of Grade(s) to select the desired grade level(s) to be included.
Print in Homeroom Order--Select to print the forms in homeroom order
Homeroom--Select assigned student homeroom teachers to print-leave empty for all
Show Sped--Sped option
Show 504--504 option
Show LEP--LEP option
Active only--This choice will list only those students who are actively enrolled as of the date selected in the date field.
As of Date--This choice will list only those students who are actively enrolled as of the date selected in the date field.
Print Comments--Select to print a student short form with each request sheet-click the button labeled "Enter Comments"
Gender--Gender of students whose records need to be edited or whose names are to be included on a report. Leave the field blank if both genders are to be included. Otherwise, click in the field to the right of Gender to select the desired gender to be included.
Ethnic--Ethnicity of students whose records need to be edited or whose names are to be included on a report. Leave the field blank if all ethnics are to be included. Otherwise, click in the field to the right of Ethnic to select the desired ethnic(s) to be included.
Teams--This selection should only be used if your school is using Team Scheduling. Leave blank if all team names are to be included into the report. Otherwise click in the field to the right of Teams to select the desired Teams(s) to view.
Counselor--Responsible for scheduling, testing, student grades and transcripts. Leave blank if all counselors are to be included into the report. Otherwise click in the field to the right of Counselor to select the desired counselor(s) to view.
Advisor--Selects students for this report based on the Advisor that have been assigned to individual students.
Special Codes--Special Codes must have been entered into the Student Master record for students. Leave blank if all special codes are to be included into the report. Otherwise click in the field to the right of Special Codes to select the desired special code(s) to view.
Exclude Special Codes - Check to exclude the special code(s) that you choose and do not want included on the report.
Program--Leave blank if all program codes are to be included into the report. Otherwise click in the field to the right of Program Codes to select the desired program code(s) to view.
Clubs--Extracurricular activities in which students may participate. Leave blank if all clubs are to be included into the report. Otherwise click in the field to the right of Clubs to select the desired club(s) to view.
Sports--Leave blank if all sports are to be included into the report. Otherwise click in the field to the right of Sports to select the desired sport(s) to view.


Click Ok to move to the next screen.

For other columns not listed in the common column list:

Choose a student(s) by placing a check in the box to the left of the student's name. You can also use the "Select All" box to the right of the Name column. Click on "Print" at the bottom of the page and you will see the box below.

Column Headers

Name--Student Name--Student's/employee's full name
Sidno--Sidno, ID--local ID of the student
GD--Grade, Grd--grade level of the student and/or course
Homeroom--Homeroom--student's homeroom teacher
Dist--District (Dist)--district in which the student is enrolled
Sch--School/Site--site code of the school in which the student enrolled
Counselor--Counselor--name of the counselor to which the student has been assigned
Advisor--name of the advisor to which the student has been assigned
Team--team to which the student has been assigned if the school is using the team scheduling method

Bottom

For Print Options:

  1. Check the first box if you want to see scheduled requests as well as unscheduled requests.
  2. Check the first box if you want to print all course offerings at the end of each student's request forms.
  3. Check the third box if you want to print the student's enrollment short form with every request form.


For Year and Credit Options:

  1. Check the first box if you want to add alternate course credits to the total credits.
  2. Check the next boxes according to the year(s) you need to print.


Click on Print Forms.

  1. Click the printer icon at the top right. Another print servlet will appear.
  2. Click the print button at the top left to print the report. Close the original print servlet window.
